The Role of Wearable Technology in Improving Patient Outcomes in Chronic Disease Management

Abstract
The increasing prevalence of chronic diseases and the demand for cost-effective healthcare have accelerated the adoption of remote patient monitoring (RPM) technologies. Wearable devices enable real-time collection of physiological data, enhancing early detection of clinical deterioration and supporting patient-centered care. This study is a narrative review that synthesizes evidence from recent literature on wearable technology and RPM, highlighting clinical applications, technological integration, benefits, challenges, ethical considerations, and future directions. Findings indicate that wearable RPM improves clinical outcomes, enhances patient engagement, and improved self-management in chronic disease care. However, challenges remain regarding interoperability; device validation, workflow integration, and data privacy. This paper contributes to existing literature by integrating technology, clinical, and implementation perspectives and by applying the technology acceptance model (TAM) and the chronic care model (CCM) as a conceptual framework to guide wearable RPM adoption. Recommendations for policy, clinical practice, research, and technology standardization are proposed.
